When your naturalization or citizenship certificate is lost, stolen, destroyed, or damaged, when USCIS printed something wrong on it, or when you legally changed your name after it was issued.

Not if you already have a valid U.S. passport, which is also proof of citizenship. For a first passport, the certificate is the usual proof, so a lost one is worth replacing first.

There is no test. Most N-565 cases are decided on the documents, though USCIS can ask you to come in.

USCIS charges a filing fee for Form N-565, paid directly to USCIS, and none when the certificate is wrong because of a USCIS error. The USCIS fee table lists the current amount.
